Diabetic Foot Screening

Our team developed a diabetic foot screening, risk stratification, and education program based upon research evidence and international consensus guidelines. This program is designed to identify diabetic patients who are at risk of developing foot ulceration that is visually illustrated by a “traffic light” system: Green = low/no risk, Orange = Moderate risk, and Red = High risk. The outcome of screening will determine which interventions are required to lower ulceration risk in the moderate and high-risk categories. All screened patients are invited to watch an educational video and a one-to-one consultation. The screening process is designed to be a repetitive process with review intervals set according to risk status.
